Records Request Checklist
The Office of Disability Services retains disability-related student records for seven years after the date of graduation. The student will need to provide permission by email granting the release of records. Please include in your email a list of items you are requesting, such as, medical letters, evaluations and/or a letter formatted by ODS indicating previously approved accommodations. The copy of the records will be sent directly to the student.
The completion of the Records Request will take approximately two to three weeks after the request date. Please note the file location or format and the current demands of the semester at the time of the request, may impact the response time. Please provide advanced notice for the request.
