Returning Student Athletes

Please read the Intrductory letter for all necessary information. The ADHD (Sports Medicine) and Publicity (Sports Information) forms, which are in PDF format must be downloaded and submitted to the their respective departments. All other forms are located in SportsWare On Line. There are directions on how to log into SportsWare by clicking on any of the forms below that are on SportsWare. This information should be completed by July 17th for Fall Sport Athletes and August 4th for Winter and Spring Athletes.
