All new incoming students (first-year and transfer) must complete the following requirements:

Step 1:

In preparation for your arrival to campus, please click on the links below to read the following information.

Step 2:

Print the Muhlenberg College Health Forms and bring to your health care provider to complete.

  • This form includes the Physical Exam Record and Immunization Record. Both the Physical Exam Record and Immunization Record must be completed by your health care provider.
  • Call your primary healthcare provider to schedule an appointment to have a physical exam completed and to have your immunization record filled out.
  • Be sure to add your full legal name and birth date to all pages of the health forms.
  • Your health care provider must complete our forms. ONLY our Muhlenberg College Health Forms will be accepted.
  • Use the opportunity of visiting your healthcare provider to discuss any health conditions you may have and how you can be prepared to manage these conditions while you are at school.
  • Before leaving your healthcare provider's office, ensure that all sections of the required forms are completed, signed, and dated where indicated.
  • Scan the completed and signed physical examination record and immunization record, save a copy on to your computer and upload the physical exam to your Pyramed Health Portal under the "Document Upload - Physical Exam" sect

Step 3:

Log into PyraMed Health Portal and complete the following forms on the PyraMed Health Portal (see below for how to access the PyraMed Health Portal).

(Click on “My Forms” tab)

  • Immunization History
    • You must manually enter the dates of immunizations on this form.
  • Medical History
  • Health Insurance
    • You will be prompted to upload your Health Insurance card within the form.
    • Please note- that completing this form and uploading your insurance card to the patient portal is not a substitute for waiving the student health insurance plan as required in Step 4.
  • Consent for Treatment, Notice of Privacy Practices, & Communication Preferences
  • Emergency Contacts

Step 4:

Student Health Insurance Plan – Enrollment or Waiver Required

**Every student must either enroll in or waive the Student Health Insurance Plan by August 1.**

Waiving the Student Health Insurance Plan: Students who are currently enrolled under a
personal or family health insurance plan and would like to waive the Student Health Insurance Plan must complete an online waiver through the link above This waiver must be completed in addition to submitting their health insurance information through the Pyramed student health portal.

Enrolling in the Student Health Insurance Plan: Enrollment through the Student Health Insurance Plan is completed through the link above. Once the enrollment process has been completed, the student will receive notification by email. If you do not receive an email notification please send an email to [email protected].

**Special note for F-1 international students: Per visa requirements, F-1 students will continue to be automatically enrolled in this new College-wide health insurance plan. Students should still expect to see the charge on their Fall billing statements. F-1 students WILL NOT need to enroll or waive this plan themselves since it will be automatic.

 

Student Health Portal Information

Health Forms must be submitted through the PyraMed Health Portal.  Please call Health Services (484-664-3199) with any concerns.

To access the PyraMed Health Portal:

  • First, access the Muhlenberg College Application Portal (login.muhlenberg.edu)
  • Enter your Muhlenberg user name and password (supplied by Informational Technology)
  • Click on the icon for PyraMed Health Portal

Refer to the “What You Need to Know about Technology @ Muhlenberg College” letter you received from Information Technology to access the Application Portal. 

Sports Medicine

For incoming varsity athlete students and also for those considering trying-out for a varsity sport, (in addition to completing all the forms above), you are required to complete additional medical forms.  See the Sports Medicine website for this required information.
